What is included

2 check ups within contract expiration (28% discount) including:

• Bitewing X-rays
• Preventative (Prophylaxis) Cleaning
• 2 Dental Exams
• 2 periapical x-rays
• Fluoride Varnish

Member Savings

Members receive 20% savings on following treatments:

• Fillings
• Crowns / Veneers / Bridges
• Extractions
• Implants and Implant Restorations
• Gingivitis cleanings
• Periodontal Maintenance cleanings
• Scaling and Root Planing
• Sealants
• Other necessary X-rays – Panoramic/full mouth (once every 5 years)

*Excluding products such as whitening kits, electric toothbrushes, etc.

Membership Pricing

Prices are figured at a 12 Month rate, and are as follows:

1st immediate family member	$375
2nd & 3rd immediate family members	additional $200
Additional immediate family members	additional $150

*Please note that to have multiple family members covered on this plan, all family members must live in the same household.
